Number of packages - 2: - package No. 1 - maintenance services for medical equipment and devices manufactured by Olympus,- package No. 2 - maintenance service of angiograph - cardiovascular system Allura Xper FD20C with Intellivue monitoring system of Philips Medical System.The subject of the contract is to provide maintenance services for medical equipment manufactured by Philips Medical System and the manufacturer Olympus being the equipment of the Dr Alfred Sokołowski Specialist Hospital in Wałbrzych. The work performed under the contract consists of servicing medical equipment and apparatus in accordance with the manufacturer's recommendations and repairs carried out depending on the needs of the Purchaser.
1. Provision of maintenance services for Olympus medical apparatus and equipment - flat rate for repairs and maintenance over a period of 36 months: 1. The works performed within the scope of the contract consist in servicing the medical apparatus and equipment in accordance with the manufacturer's recommendations as well as repairs carried out depending on the Ordering Party's needs and include: - periodic and preventive inspections specified by the manufacturer,- diagnoses and repairs including replacement of damaged parts,- substitute devices for the time of repairs,- emergency visits,- technical assistance and remote service,- updates,- audits, practical and technical training,- travelling.The Contractor declares that within the scope of the above-mentioned contract description he has an appropriate qualified staff.2.The Contractor shall provide maintenance services of the medical equipment and apparatus and shall employ personnel with qualifications and professional experience specified by manufacturers (importers, distributors) in accordance with the Medical Devices Act of 20 May 2010 (Journal of Laws 2020, item 186).3, Provision of maintenance services - agreement for a period of 36 months.Monthly remuneration for the performance of the contract will be paid in 36 equal installments.Diagnostic equipment manufacturer Olympus: device name; type; serial number; year of manufacture; manufacturer; organizational unit: 1. bronchofiberoscope BF-1T60 2342511 2013 Olympus Medical Systems Co. Provision of maintenance service for Angiograph Cardiovascular System Allura Xper FD20C with Intellivue monitoring system by Philips Medical System - lump sum for repairs and services during 24 months:1. As part of the contract, the work performed consists of servicing the apparatus and medical equipment in accordance with the manufacturer's recommendations and repairs carried out depending on the needs of the Employer and include: - periodic and preventive maintenance specified by the manufacturer,- checking the technical condition of the system and key components,- scheduled maintenance at regular intervals based on the manufacturer's recommendations - the manufacturer's requirements, min. 2 times a year for the main system,- trips, diagnoses, repair services and equipment calibrations,- remote diagnostics,- unlimited replacement of all damaged spare parts, if necessary,- software and design modifications recommended by the manufacturer, improving work efficiency and safety,- priority in handling requests,- repair time without the use of spare parts - up to 3 working days,- repair time with the use of spare parts - up to 5 working days.The Contractor declares that he has adequate qualified staff within the above scope of the order description.The scope of work depends on the requirements of the medical device manufacturer.
