Description

The subject of the order is to make a successive delivery called "Medicinal products divided into 21 tasks" listed in tasks 1-20 and medical products listed in task 21 to the Hospital Pharmacy in the Centre for Occupational Medicine and Rehabilitation ATTIS Sp. z o. o. in Warsaw. The detailed description of the subject of the order, i.e. the list of assortment, its parameters and required quantity is contained in the assortment and price form for tasks no. 1-21, which constitutes the attachment no. 2 to j ToR.
Task no. 1 - item 1-448,If a given medicinal product is included in the list of reimbursed medicines, foodstuffs for special nutritional purposes and medical devices: Annex C - "Medicines used within the framework of chemotherapy in the whole range of registered indications and purposes and in the indication of a specific clinical condition", the Contractor is obliged to offer the product covered by the aforementioned list. When calculating the price, the Contractor must take into account the provisions of art. 9 item 2 of the Act of 12 May 2011 on reimbursement of medicines, foodstuffs for special nutritional purposes and medical devices (i.e. Journal of Laws of 2020, item 357) and the notice of the Minister of Health on the list of reimbursed medicines, foodstuffs for special nutritional purposes and medical devices, valid on the day of submitting the offer.In case of offering a medicine, foodstuff for special nutritional purposes or medical device covered by the refund, its price may not be higher than the official selling price specified in the notice of the Minister of Health on the list of reimbursed medicines, foodstuffs for special nutritional purposes and medical devices, current as of the date of submitting the offer. - For an Economic Operator not conducting wholesale trade, the price offered shall not be higher than the official selling price of the product constituting the basis for the limit, plus a margin not higher than the official wholesale margin.In the case of removing a medicine from the reimbursement notice of the Minister of Health during the term of the contract, the Ordering Party reserves the right to terminate the contract, unless the entity authorized to wholesale trade offers an equivalent contained in the reimbursement notice of the Minister of Health, whose gross wholesale price shall not be higher than the gross wholesale price of the medicine whose deliveries are the subject of the contract and not higher than the funding limit valid on the day of the change. With regard to all medicines, the Ordering Party requires the EAN code to be provided in the offer. Task No. 2 - items 1-28,If a given medicinal product is included in the list of reimbursed medicines, foodstuffs for special nutritional purposes and medical devices: Annex C - "Medicines used within the framework of chemotherapy in the whole range of registered indications and purposes and in the indication of a specific clinical condition", the Contractor is obliged to offer the product covered by the above mentioned list. Task no. 12 - item 1,If a given medicinal product is included in the list of reimbursed drugs, foodstuffs for special nutritional purposes and medical devices: Annex C - "Medicines used within the framework of chemotherapy in the whole range of registered indications and purposes and in the indication of a specific clinical condition", the Contractor is obliged to offer the product covered by the above mentioned list. Task no. 12A - item 1-2, If a given medicinal product is included in the list of reimbursed drugs, foodstuffs for special nutritional purposes and medical devices: Annex C - "Medicines used within the framework of chemotherapy in the whole range of registered indications and purposes and in the indication of a specific clinical condition", the Contractor shall offer the product covered by the above mentioned list.
