Description
1 The subject of the order is the supply of personal protective equipment under the project "Development and maintenance of rescEU CBRN stockpiles in Poland" - stage 1 . 2 The order has been divided into ten Parts. (3) A detailed description of the subject matter of the order, in particular, specification of technical and functional parameters of the subject matter of the order, with a detailed division into Parts, constitutes Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the order, individually for each Part of the order, are specified in Attachment No. 2A - 2J to the SWZ - "Draft contractual provisions".
(1) Under Part 1 of the order, the Contractor shall be obliged to provide air purification devices with full face mask, in the number of 500 sets (sets), in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the order". (2) Notwithstanding the service of providing the equipment referred to in paragraph 1 above, the Contractor shall further be obliged to:A. conduct a one-day training course on the operation, maintenance and use of the supplied equipment for no less than 6 and no more than 9 employees of the Ordering Party or other persons designated by the Ordering Party, B. permanently and visibly mark each package of each equipment with the logo of the European Union flag, together with a statement on the method of financing: "Funded by the European Union", the design of which is attached as Appendix No. 1A to the SWZ - "Model Logo", subject to:a. the dimensions of the EU flag - the dimensions of the EU flag may not be smaller than: 10.5 cm (height) x 16 cm (width), with a properly visible statement on the method of financing, 1 cm (height), with a properly visible statement on the method of financing - concerning packages with dimensions smaller or equal to 21 cm x 29.7 cm,b. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to each other, c. The need to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging - clear visibility of the marking, d. The need to maintain the color scheme of the logotype and the statement on the method of financing (compliance with the pattern), e. The Employer allows the marking of the equipment packaging with the use of stickers, stickers and similar materials, if they provide a permanent connection with the equipment packaging, f. The Employer allows the marking of the equipment packaging only if the equipment in question is pre-packaged by the manufacturer, i.e. when the equipment in question packed in a unit package is customarily transported in a collective package. In particular, the Ordering Party does not allow unit packaging to be combined into bulk packaging, or bulk packaging customarily packaged by the manufacturer, into larger bulk packaging (packaging "pre-packaged" by the Contractor) in order to avoid the Contractor's obligation to label each unit and/or bulk packaging of the equipment, C. provide the Ordering Party with a quality guarantee for the equipment supplied until at least December 31, 2026, under which the Contractor will be obligated to:a. rectify defects and faults in the equipment, b. Perform or ensure the performance of periodic inspections of the equipment or its individual components, as required and/or recommended by the manufacturer of the equipment, including the provision of ongoing (during the period of operation) and/or periodic (in particular, during or as a result of a periodic inspection) replacement and/or replenishment of consumables and/or replacement parts, at the Contractor's sole expense and under the terms and conditions specified in Appendix No. 2A to the SWZ - "Draft Contractual Provisions". (3) A detailed description of the subject matter of the order, in particular, determination of the technical and functional parameters of the subject matter of the order is provided in Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the order are specified in Appendix No. 2A to the SWZ - "Draft contractual provisions". (1) Under Part 2 of the order, the Contractor shall be obliged to supply full-face masks for filters and filter absorbers with RD 40 thread, in the quantity of 3,000 pieces, in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the order". (2) Notwithstanding the provision of the PPE referred to in paragraph 1 above, the Contractor shall further be obliged to:A. conduct a one-day training course on the operation, maintenance and use of the supplied PPE for no less than 6 and no more than 9 employees of the Ordering Party or other persons designated by the Ordering Party, B. permanently and visibly mark each package of each PPE with the logo of the European Union flag, together with a statement on the method of financing: "Funded by the European Union", the design of which is attached as Attachment No. 1A to the SWZ - "Logo design", subject to:a. the dimensions of the EU flag - the dimensions of the EU flag may not be smaller than: 10.5 cm (height) x 16 cm (width), with a properly visible statement on the method of financing, 1 cm (height), with a properly visible statement on the method of financing - concerning packaging with dimensions less than or equal to 21 cm x 29.7 cm,b. the need to maintain the proportions of the logotype and the statement on the method of financing in relation to each other, c. the need to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging - clear visibility of the marking, d. the need to maintain the color scheme of the logotype and the statement on the method of financing (compliance with the pattern), e. the Employer allows marking of the PPE packaging with the use of stickers, stickers and similar materials, if they provide a permanent connection with the PPE packaging, f. the Employer allows marking of the PPE packaging only if the PPE in question are pre-packaged by the manufacturer, i.e. if the PPE in question packed in a unit package is customarily transported in a unit package. In particular, the Ordering Party does not allow the combination of unit packs into bulk packs or bulk packs customarily pre-packaged by the manufacturer into larger bulk packs (packs "pre-packaged" by the Contractor) in order to avoid the Contractor's obligation to label each unit and/or bulk pack of PPE, C. provide the Ordering Party with a quality guarantee for the supplied PPE until at least December 31, 2026, under which the Contractor will be obligated to:a. rectify defects and faults in the PPE, b. perform or ensure the performance of periodic inspections of the PPE or its individual components, as required and/or recommended by the manufacturer , including the provision of ongoing (during the period of operation) and/or periodic (in particular, during or following a periodic inspection) replacement and/or replenishment of consumables and/or replaceable parts, at the sole expense of the Contractor and under the terms and conditions specified in Appendix No. 2B to the SWZ - "Draft contractual provisions". (1) Under Part 3 of the order, the Contractor shall be obliged to provide equipment for transporting infectious patients, in the number of 50 units, in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the order". (2) Notwithstanding the provision of the equipment referred to in paragraph 1 above, the Contractor shall further be obliged to:A. conduct a one-day training course on the operation, maintenance and use of the supplied equipment for no less than 6 and no more than 9 employees of the Ordering Party or other persons designated by the Ordering Party, B. permanently and visibly mark each package of each equipment with the logo of the European Union flag, together with a statement on the method of financing: "Funded by the European Union", the design of which is attached as Appendix No. 1A to the SWZ - "Model Logo", subject to:a. the dimensions of the EU flag - the dimensions of the EU flag may not be smaller than: 10.5 cm (height) x 16 cm (width), with a properly visible statement on the method of financing, 1 cm (height), with a properly visible statement on the method of financing - concerning packages with dimensions smaller or equal to 21 cm x 29.7 cm,b. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to each other, c. (3) A detailed description of the subject matter of the order, in particular, determination of the technical and functional parameters of the subject matter of the order is provided in Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the contract are specified in Appendix No. 2C to the SWZ - "Draft contractual provisions". (1) Under Part 4 of the contract, the Contractor shall be obliged to supply:a. Type 4 suits, in the amount of 5,000 pieces, b. Type 5 suits in the amount of 55,000 pieces, in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the contract". (3) A detailed description of the subject matter of the order, in particular, determination of the technical and functional parameters of the subject matter of the order is provided in Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the order are specified in Annex 2D to the SWZ - "Draft contractual provisions". (1) Under Part 5 of the order, the Contractor shall be obliged to provide biological protection suits with filtering apparatus, in the number of 200 sets (sets), in accordance with the technical specification specified in Appendix No. 1 to the SWZ - "Description of the subject of the order". (1) Under Part 6 of the order, the Contractor shall be obliged to supply type 1b suits, in the quantity of 2,000 pieces in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the order". In particular, the Ordering Party does not allow unit packages to be combined into bulk packages, or bulk packages customarily pre-packaged by the manufacturer, into larger bulk packages (packages "pre-packaged" by the Contractor) in order to avoid the Contractor's obligation to label each unit and/or bulk package of PPE, C. provide the Ordering Party with a quality guarantee for the supplied PPE until at least December 31, 2026, under which the Contractor will be obligated to:a. rectify defects and faults in the PPE, b. perform or ensure the performance of periodic inspections of the PPE or its individual components, as required and/or recommended by the manufacturer , including the provision of ongoing (during the period of operation) and/or periodic (in particular, during or following a periodic inspection) replacement and/or replenishment of consumables and/or replacement parts, at the sole expense of the Contractor and under the terms and conditions specified in Appendix No. 2I to the SWZ - "Draft Contractual Provisions". (3) A detailed description of the subject matter of the order, in particular, determination of the technical and functional parameters of the subject matter of the order is provided in Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the contract are specified in Appendix No. 2I to the SWZ - "Draft contractual provisions". (1) Under Part 10 of the contract, the Contractor shall be obliged to provide:a. chemical protection gloves in the amount of 10,000 pairs, b. biological protection gloves in the amount of 10,000 pairs, c. chemical / biological protection boots in the amount of 5,000 pairs, in accordance with the technical specifications specified in Appendix No. 1 to the SWZ - "Description of the subject of the contract". (2) Notwithstanding the provision of PPE referred to in paragraph 1 above, the Contractor shall further be obliged to:A. permanently and visibly label each package of each PPE with the logo of the European Union flag, together with a statement of funding: "Funded by the European Union", the design of which is attached as Attachment No. 1A to the SWZ - "Logo design", subject to:a. the dimensions of the EU flag - the dimensions of the EU flag may not be smaller than: 10.5 cm (height) x 16 cm (width), with a properly visible statement on the method of financing, 1 cm (height), with a properly visible statement on the method of financing - concerning. packaging with dimensions less than or equal to 21 cm x 29.7 cm,b. the need to maintain the proportions of the logotype and the statement on the method of financing in relation to each other, c. the need to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging - clear visibility of the marking, d. the need to maintain the color scheme of the logotype and the statement on the method of financing (compliance with the pattern), e. the Employer allows marking of the PPE packaging with the use of stickers, stickers and similar materials, if they provide a permanent connection with the PPE packaging, f. the Employer allows marking of the PPE packaging only if the PPE in question are pre-packaged by the manufacturer, i.e. if the PPE in question packed in a unit package is customarily transported in a unit package. In particular, the Ordering Party does not allow the combination of unit packs into bulk packs or bulk packs customarily pre-packaged by the manufacturer, into larger bulk packs (packs "pre-packaged" by the Contractor) in order to avoid the Contractor's obligation to label each unit and/or bulk pack of PPE, B. to provide the Ordering Party with a quality guarantee for the supplied PPE until at least December 31, 2026, under which the Contractor will be obliged to rectify defects and faults in the PPE, at the Contractor's sole expense and under the terms and conditions specified in Appendix No. 2J to the SWZ - "Draft Contractual Provisions". (3) A detailed description of the subject matter of the order, in particular, determination of the technical and functional parameters of the subject matter of the order is provided in Appendix No. 1 to the SWZ - "Description of the subject matter of the order". (4) The terms and conditions of performance of the subject matter of the order are specified in Appendix No. 2J to the SWZ - "Draft contractual provisions".
