An Application form to confirm qualification including the information of designated technical
personnel for placement (Appendix 1 and other attachments), documents related to general
evaluation system (Form 4) (1 document), and a Cost estimate calculated by estimate utilization
method (Form 2) (1 document).
iii) Submission method:
Bidding takes place via the online bidding system. However, if the combined size of the attached
files exceeds 3MB, with permission, the application and necessary documents may be submitted
via post or brought in person to the following address along with submission of the application
form via the online bidding system:
