United States - Fermilab - Four village houses upgrade

INTRODUCTION
This project intends to obtain a qualified Subcontractor to perform interior repairs including bathroom demolition, bathroom reinstall, replacement of galvanized pipe, painting of kitchen cabinets, replacement of countertops, interior painting, installation of light fixtures, and replacement of flooring at Village House Potawatomi 14 at Fermi National Accelerator Laboratory (Fermilab) located in Batavia, Illinois. Fermilab is operated by the Fermi Research Alliance (FRA).  FRA will provide a Task Manager / Construction Coordinator (TM/CC) to provide oversight of the project.

The purpose of this statement of work (SOW) is to obtain the supervision, expertise, labor, materials, and equipment in relation to the work indicated below.

SCOPE OF WORK

The Subcontractor shall perform the following:

Bathroom Demolition

Mobilize to Potawatomi 14 in the Fermilab Village.
Cover door openings with plastic sheeting (or similar) to prevent the spread of dust to other areas.
Ensure that water to the bathroom is secured, as necessary.
Remove all currently installed fixtures, flooring, wall tile, and cabinets from the bathroom.
Remove all existing galvanized pipe.
Dispose of removed items and debris in dumpster provided by FRA.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Bathroom Remodel

Mobilize to Potawatomi 14 in the Fermilab Village.
Replace all existing galvanized pipe in the bathroom with copper pipe.
Adapt and renovate any plumbing positions as necessary for installation of new fixtures.
Determine if any changes are required for electrical outlets or fixtures.
Perform Lock Out / Tag Out (LOTO) of any electrical outlets or fixtures as needed.
Install new light fixture, fan, and outlets equivalent to what is currently installed in the bathroom.
Install new vinyl wall and floor tile equivalent to that currently installed.
Install new single sink bathroom vanity (grey cabinet with white vanity sink) and verify leak tight.
Install new standard acrylic white bath tub / shower surround and verify leak tight.
Install new standard white, round toilet and verify leak tight.
Install standard satin nickel finish towel bar, paper holder, and shower curtain bar.
Dispose of any garbage / debris in dumpster provided by FRA.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Kitchen Countertop Demolition

Mobilize to Potawatomi 14 in the Fermilab Village.
Ensure that water to the kitchen is secured, as necessary.
Disconnect sink drains and water lines from faucet and ensure there is no connection to countertop.
Remove kitchen countertop taking care to preserve kitchen cabinets.
Clean any remaining adhesive from cabinets.
Dispose of old countertop in FRA provided dumpster.

Paint Kitchen Cabinets

Mobilize to Potawatomi 14 in the Fermilab Village.
Remove all hardware and doors from kitchen cabinets.
Prep and clean cabinets for painting by using a degreaser and lightly sanding, as required.
Prime cabinets to ensure paint adheres to the surface.
Paint the cabinets using a minimum of two coats Benjamin Moore EcoSpec in color #1600 Timber Wolf.
Reinstall doors and hardware.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Install Kitchen Countertop

Mobilize to Potawatomi 14 in the Fermilab Village.
Measure the installation area for the countertop to ensure proper fit.
Cabinets should be laminate in Wilsonart White Carrara 4924-38 or equivalent.
Dry fit the countertop and scribe to fit any irregularities in the wall or cabinets.
Cut or sand along scribe line, as necessary, to ensure proper fit.
Fasten countertops to cabinets.
Caulk any seams or gaps.
Reinstall sink and caulk any seams or gaps.
Reattach sink drains and faucet.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Carpet Removal and Installation

Mobilize to Potawatomi 14 in the Fermilab Village.
Remove existing carpet and pad in all currently carpeted rooms.
Dispose of carpet and pad in FRA supplied dumpster.
Prep floors as necessary to ready them for carpet installation (staple removal, adhesive removal, cleaning, etc.).
Install new carpet and pad (Dream Weaver, Rock Solid III, Dark Platinum).
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Laminate Flooring Removal and Installation

Mobilize to Potawatomi 14 in the Fermilab Village.
Remove existing non-carpet flooring and underlayment (as applicable) in all rooms (except bathroom).
Dispose of old flooring in FRA supplied dumpster.
Prep floors as necessary to ready them for laminate floor installation (staple removal, adhesive removal, cleaning, etc.).
Install new laminate flooring and underlayment (Aladdin Commercial AH080 in color 910 Drumhill).
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Galvanized Pipe Replacement

Mobilize to Potawatomi 14 in the Fermilab Village.
Ensure that water is isolated to the building.
Remove all existing galvanized pipe in the building.
Dispose of removed pipe in the dumpster provided by FRA.
Install copper pipe of the same diameter to replace the existing galvanized pipe.
Restore water flow to the building and validate that all newly installed pipe is leak tight.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Interior Paining

Mobilize to Potawatomi 14 in the Fermilab Village.
Tarp, mask, and prepare building for interior paint.
Scrape peeling paint and dispose as required.
Patch all holes, cracks, and imperfections prior to painting.
Paint all walls with a minimum of two coats satin paint in color to be determined.
Paint all ceilings with a minimum of two coats Benjamin Moore Eco Spec Interior Latex eggshell paint in color to be determined.
Paint all trim and interior doors with a minimum of two coats Benjamin Moore Eco Spec Interior Latex semi gloss paint (color to be provided).
Paint exterior front and back doors with a minimum of two coats Benjamin Moore Eco Spec Interior Latex semi gloss paint.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Light Replacement

Mobilize to Potawatomi 14 in the Fermilab Village.
Perform Lock Out / Tag Out (LOTO) of lights as required for installation.
Remove all non-functional light fixtures from the building.
Replace non-functional light fixtures with new fixtures. 
Remove LOTO and validate that fixtures are functional.
Dispose of removed lights per FRA guidelines.

Dumpster will be provided by FRA for disposal of waste.

Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Window Replacement

Mobilize to Potawatomi 14 in the Fermilab Village.
Remove existing windows and trim from the dining room and front of the house. 
Dispose of old window and trim in FRA supplied dumpster.
Prepare opening for installation of new window.
Install new windows of the same style as currently installed and ensure that it properly sealed.
Reinstall trim and patch paint where required.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.

Add 220 V Power Supply

Mobilize to the laundry closet of Potawatomi 14 in the Fermilab Village.
Perform LOTO as required to prevent electrical shock.
Select a breaker that can support 220 V outlet (i.e. 30 amp, double pole).
Drill / cut holes to set electrical box and cables.
Install electrical box.
Feed electrical wire (10 gauge) from breaker box to the electrical box previously installed.
Attach wires to outlet and ensure that the outlet is grounded.
Remove LOTO and ensure that there is power to the outlet.
Perform close out of job site.

Ensure all tools, equipment, and waste are removed.
Perform turnover / closeout of job with TM/CC.
