Description

Group insurance against accidents (NNW) and civil liability (OC) of students of all types of studies at the Medical University of Gdańsk (for 36 months). Detailed description of the subject matter of the contract is specified in appendix 3 to the ToR. In the present procedure the Contracting Authority shall demand from the Economic Operators to pay a deposit in the amount of PLN 10 000 (say: ten thousand 00/100 PLN).
In order to confirm that the Economic Operator meets the conditions for participation in the procedure and that there are no grounds for exclusion, a statement in the form of a uniform European contract document (hereinafter referred to as JEDZ), drawn up in accordance with the specimen in annex 2 to the ToR, shall be submitted together with a tender. The information contained in the declaration shall constitute a preliminary confirmation that the Economic Operator is not subject to exclusion. The single document (SINGLE) shall, on pain of nullity, be drawn up in electronic form and bear a qualified electronic signature. An Economic Operator who invokes the resources of other entities in order to prove the absence of grounds for exclusion shall also submit uniform documents concerning those entities on the ONE form, which must be in the form of an electronic document signed with a qualified electronic signature by each of them to the extent that each Economic Operator proves the absence of grounds for exclusion. A similar requirement shall apply to the ONE when Economic Operators are competing jointly for the contract. Those documents shall confirm the absence of grounds for exclusion. In the case of joint procurement by economic operators, the declaration referred to in paragraph 1(1) above shall be made by each of the economic operators jointly applying for the contract. The declaration shall confirm the fulfilment of the conditions for participation in the procedure and the absence of grounds for exclusion. Where an Economic Operator invokes the resources of other entities, the declaration referred to in paragraph 1 point 1 of Chapter V of this chapter concerning those entities shall be made in order to demonstrate that there are no grounds for exclusion. Before awarding the contract, the Contracting Authority shall, within the scope of proving by the Economic Operator that there are no grounds for exclusion, call upon the Economic Operator whose tender was the highest rated to submit the following statements or documents within a specified time limit, not shorter than 10 days, valid at the date of submission: information from the National Criminal Register within the scope defined in art. 24 item 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of tenders; an extract from the relevant register or from the central register and information on business activity, if separate regulations require an entry in the register or register to confirm the absence of grounds for exclusion under art. 24 item 5 point 13 of the Act. The Economic Operator's declaration that no final court verdict or final administrative decision on arrears in payment of taxes, fees or social or health insurance contributions has been issued against him or her, if such a verdict or decision has been issued, documents confirming payment of such dues together with interest or fines, if any, or conclusion of a binding agreement on payment of such dues; the Economic Operator's declaration that he or she has not been declared unsuccessful as a preventive measure of the ban on competing for public contracts.If the Economic Operator has a seat or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in item 4 point 1 and item 4 point 2 of the ToR, he shall submit the documents according to the provisions of the ToR Chapter V.
